PFS Claim Form


The PFS Claim Form is used by Massif Customer Service to notify PFS and Massif’s Warehouse Team of shipping issues that arise. Upon receipt of this form, we ask that PFS conducts an investigation into the matter and then sends a detailed response with its findings. If applicable, we ask that PFS also shares how the issue will/can be prevented moving forward. Massif hopes to receive a response within 3 business days.
